CLIENT CARE COORDINATOR

Position Overview The client care coordinator serves as a primary contact for our senior clients and their family members at all stages of the service relationship including pre-service assessments, service plan development, and regular in-home quality assurance visits. The client care coordinator reports directly to the general manager and offers critical insights to improve services for scores of senior clients.

Location The desk for this position is in our downtown Hillsboro office. The job, however, includes many field visits in clients' homes throughout Washington County .
